Coordination

Coordination is ensuring that the patient, family, caregivers, and professionals understand the patient's goals and objectives. Based on these objectives, care is able to be provided to orient toward these goals. By coordinating these conversations, palliative care can allow patients and their families to focus on what truly matters most to them, not trying to coordinate details.
